Consent:

I, the undersigned parent/guardian of the above-named participant, hereby grant my consent for them to participate in the Jackrabbit program. I understand the inherent risks associated with track and field activities and affirm that my child is physically fit to participate.

Medical Information:

I hereby authorize the administration of any necessary first aid or emergency medical treatment in the event of injury or illness. I also acknowledge that I am responsible for any medical costs incurred.

Photo/Video Release:

I grant permission for the use of photographs and videos of my child taken during team activities for promotional or informational purposes related to Coach Andre Enterprises & Jackrabbit Track

Liability Waiver:

I release Jackrabbit Track Georgia, its coaches, and volunteers from any liability for injuries or accidents that may occur during team activities.

ALL MONIES PAID ARE NON-REFUNDABLE UNDER ANY CIRCUMSTANCES REGARDLESS OF PARTICIPATION LEVEL OR TIME. THERE WILL BE NO CREDIT AVAIBLE TO USE TOWARDS ANY OTHER JACKRABBIT TRACK GEORGIA ACTIVITIES OR EVENTS
